A Bold Leap Forward For On Lok

By: Nicole Torres, Chief Operating Officer

A Groundbreaking Achievement

We’re thrilled to share some incredible progress on one of the most exciting initiatives in our organization’s history, the PACE Expansion (PACE X) Project! This bold step forward is a cornerstone of our strategic vision to grow On Lok’s PACE program and bring our compassionate, comprehensive care to even more seniors in our communities.

What is PACE X?

PACE X is the name of our expansion project that includes two brand-new On Lok PACE centers — one in Union City and one in South San Jose. These centers will significantly increase our capacity to serve seniors in southern Alameda and Santa Clara counties. And here’s the best part: both centers are expected to open in early 2026!

Construction Milestones
  • South San Jose (151 Martinvale Lane): We’ve hit a major milestone! Construction is nearly complete, and we’re wrapping up inspections and regulatory requirements.

  • Union City (2600 Central Avenue): PG&E completed essential work earlier this month and our construction is completing their remaining work. Union City will initially open as an Alternative Care Setting (ACS), offering clinic services for our On Lok PACE Peralta Center participants. This transition also supports our move away from the Mowry clinic and Fremont Administrative Office.

Celebrating Our Collective Wins

This expansion is a true team effort. 22 department-specific workgroups are collaborating across On Lok to ensure we’re ready to open our doors with excellence. Your dedication and teamwork are making this dream a reality!

Staff Excitement is Building

Over 25 employees have already applied to transfer to the new centers, a clear sign of enthusiasm and belief in our mission! We’re so grateful to see our experienced team members stepping up to help shape the future of PACE in the South and East Bay.

Special Thanks

A huge shoutout to Janice Fujii, Director of Program Management, for her outstanding leadership in guiding this project forward. Your vision and coordination are helping us bring PACE X to life!

Let’s keep the momentum going!

We’re building more than just new centers — we’re building a stronger, more connected On Lok community ready to serve even more seniors with heart and excellence.

  • Stay tuned for more updates, including a FAQ, as we move closer to opening day.

Let’s celebrate this milestone together — the future is bright, and 2026 is just the beginning!

Walking Together for a World Without Alzheimer’s

By: Charlene Elefante, Senior Manager Community Engagement

On Lok Teams Step Up for Alzheimer’s Awareness

Silicon Valley Walk to End Alzheimer’s

The South East Bay team proudly participated in the 2025 Silicon Valley Walk to End Alzheimer’s on Saturday, October 11, at Cunningham Park in San Jose. Staff from all three centers came out to represent On Lok with great energy and compassion. Our Enrollment team also represented by staffing the On Lok information table during the exhibitor portion of the event—connecting with community members, sharing information about PACE, and helping raise awareness about our services for older adults and their families.

San Francisco Walk to End Alzheimer’s

The following week, On Lok was once again on the move—this time at the San Francisco Walk to End Alzheimer’s on Saturday, October 18. Led by Manuel Raneda, staff, families, and friends from across the organization joined together to walk in solidarity and hope. Manuel extends his heartfelt thanks to everyone who signed up, volunteered, donated, or simply came out on the day to help spread awareness.

Growing On Lok PACE to Serve More Seniors

Together, our participation in both the Silicon Valley and San Francisco walks reflects On Lok’s deep commitment to the Alzheimer’s Association’s vision of a world without Alzheimer’s. This community presence also directly supports our FY26 priority: Growing On Lok PACE to Serve More Seniors—by building visibility and trust in the communities we serve, we reach more families who can benefit from On Lok’s comprehensive care and support for aging adults. Every step we take helps us connect with more seniors, share our mission, and continue expanding access to PACE across the Bay Area.
