On Lok PACE East San Jose Center
How did you come to work at On Lok: I worked at a Residential Care Facility for the Elderly (RCFE) for more than 10 years before coming to On Lok. Based on a recommendation from a friend, I applied for a Home Care Caregiver position at On Lok in 2018 and have been here ever since.
What do you like most about your job: I like how the staff are friendly and willing to help each other. I love that supervisors show genuine care for their employees.
I like: singing songs whenever I get the chance.
I know: how to communicate well with participants and their families.
I could not do without: my rewarding work at On Lok.
In my free time: I like to dance for fun and exercise.
My superpower is: is patience.

Thanks to your feedback in our last Employee Engagement Survey, action was taken across the organization, including the launch of Appreciation in Action, making recognition more accessible, shared, and part of our everyday work.
Building on that commitment to listening, we’re excited to introduce On Lok Check-In, our first Employee Engagement short survey. It’s quick and simple; just 6 questions that take about 60 seconds to complete. This brief check-in helps us understand what’s working well and where support may be needed as we continue navigating change together.
You can take part from February 9 to February 15, 2026. The survey will be easy to access. Please look for it in your email, via text message, or through QR codes available in your centers and shared in spaces like Slack.
On Lok Check-In does not replace our annual Employee Engagement Survey. It will complements it by giving us a lighter, more frequent way to listen. Your voice matters, and your honest feedback helps guide meaningful action.

The All Hands meeting provides a dedicated format for On Lok CEO, Grace Li, to share about our strategic direction and foster alignment across the organization.
Questions submitted in advance to employeecommunications@onlok.org will be answered before the general Q&A, if there is enough time.

February 24, 2026, marks an exciting milestone for On Lok as we officially open our new On Lok PACE centers in Union City and South San Jose, where we made significant financial and strategic investments in communities where many older adults face unmet care needs.
These new centers represent more than brick and mortar. They reflect a long-term commitment to expanding access to integrated, community-based care in southern Alameda and Santa Clara counties, areas with growing senior populations and increasing demand for coordinated medical, social, and supportive services.
Through thoughtful capital investment, operational planning, and regulatory readiness, these sites are designed to expand our capacity to serve more participants while maintaining the high standards of care that define the PACE model. Each center includes fully equipped exam rooms, rehabilitation and therapy spaces, welcoming day health areas, and dedicated workspaces to support interdisciplinary care teams.
This expansion allows On Lok to:
Serve more older adults who wish to remain safely in their homes
Support family caregivers seeking comprehensive, coordinated care options
Strengthen our presence in communities where awareness of PACE continues to grow
As we open our doors, we also invite our staff and the broader community to see the impact of all the hard work firsthand.
We are offering guided tours every weekday from 10:30 a.m. to 1:30 p.m. Staff are encouraged to visit, refer family and friends, or invite community partners who may benefit from learning more about On Lok PACE. Seeing the centers in person is the best way to understand how this investment translates into care, connection, and dignity for older adults.
Opening these centers is both a celebration and a responsibility. It represents our continued commitment to meeting the evolving needs of Bay Area seniors and to ensuring that more families know how PACE helps seniors age with dignity.
We look forward to welcoming you.
For questions regarding the center tours, please reach out to the Senior Manager of Community Engagement, Charlene Elefante.
