How did you come to work at On Lok: One of my close friends told me about On Lok and suggested that I would be a great fit. At the time, I wasn’t familiar with On Lok or the PACE program, so I researched the organization and was truly inspired by its mission, vision, and the incredible work being done in the community. Fortunately, there was an opening in the Regulatory Affairs and Compliance department, and after a series of interviews, I was offered the position. My current role is the Regulatory Affairs & Compliance Manager for PACE.

What do you like most about your job: I enjoy audits because they offer a fresh perspective. When we get used to our daily routines, it’s easy to miss certain blind spots. Internal and external audits provide a different lens that helps us identify areas for improvement and strengthen compliance. I also believe that audits are an opportunity to showcase the great work we consistently do daily, highlighting our ongoing efforts and commitment to quality and compliance. In regulatory affairs and compliance, we may not provide direct care, but “we help the people help the people” by ensuring they follow regulations and deliver the best possible services to our participants.

Please share a little about you:

I like: backpacking, hiking, long-distance running, all kinds of adventures, pilates, strength training, going to concerts, watching stand-up comedy, and traveling.

I know: that every policy eventually needs…another policy.

I could not do without: my family and my fur baby, Boomer.

In my free time: I enjoy watching cinemas and shows, reading actual books (not P&P for a change, haha), and trying different kinds of food.

My superpower is: being super organized and incredibly patient. I rarely get stressed because I believe everyone is here to do their best. When challenges arise or people react in certain ways, I try to understand their perspective. Kindness and compassion are an integral part of my superpower.

See It. Say It. Secure It.

By: Mark Mayeda, Compliance Officer

Together, We Keep On Lok Secure

November is Corporate Compliance Month, and this year’s theme — “See It. Say It. Secure It.” — reminds us that protecting our participants and our organization is everyone’s responsibility.

At On Lok We See it, Say it, and Secure it.
Compliance Starts With You

If you observe or suspect a compliance concern — including a potential privacy incident, unauthorized access to information, or improper handling of participant data — please do not attempt to investigate. Instead, report your concern immediately so the appropriate review can occur.

Report to: compliance@onlok.org Anonymous reporting, available 24/7: https://www.mycompliancereport.com/report?cid=LOK

When reporting, please remember the Four W’s:
  • Who: Whose PHI (Protected Health Information) was or may have been involved?

  • What: What events or actions prompted your concern?

  • When: When did the event occur?

  • Where: Where did the event take place?

Examples of compliance breech.

1. Privacy Concern During Care Scenario: A staff member overhears another team member discussing a participant’s medical condition in the hallway, where other participants and visitors can hear.

What to Do: If comfortable, motion your colleague to a confidential space and remind her/him/they, that when discussing medical conditions, Protected Health Information (PHI), to lower their voice and/or to move to a more private area. You may also report the incident to compliance@onlok.org (or anonymously online) so the Compliance Team can follow up appropriately.

Why It Matters: Even well-intentioned conversations can unintentionally expose Protected Health Information (PHI). Reporting helps prevent future breaches and reinforces On Lok’s culture of privacy and respect for maintaining Participant confidentiality.

2. Lost or Misplaced Device Scenario: A nurse realizes she left her work-issued tablet, which contains participant records, in her car overnight. The next morning, the tablet is missing.

What to Do: She immediately reports the incident to Security and Compliance instead of trying to locate or investigate it herself. Everyone at On Lok should immediately report a lost/stolen device to: it.servicedesk@onlok.org, or give the Security team a call at (415) 292-8884.

Why It Matters: Prompt reporting allows On Lok’s IT and Compliance teams to secure the device, prevent unauthorized access of PHI and On Lok sensitive data, and meet required breach notification timelines with our federal and state regulators. Please note that On Lok is required to report to the Department of Health Care Services (DHCS) any suspected or actual breach of Participant PHI within 24 hours.

3. Holiday Event Photos Scenario: During a winter celebration, a staff member takes group photos that include Participants and posts them to social media without prior consent.

What to Do: Another team member notices the post and reports it to compliance@onlok.org for review.

Why It Matters: Even cheerful moments can lead to PHI violations if participants are identifiable and proper consent wasn’t obtained. Being mindful — especially during holidays and community events — helps everyone protect our Participant's privacy.

Non-Retaliation Reminder: On Lok’s Code of Conduct includes a non-retaliation policy. You are protected when reporting concerns in good faith — thank you for helping keep our community secure and ethical.
