Oct 24, 2025

Now is the time you can enroll, change, or waive your benefit plan options and add or remove dependents from your coverage.
No changes are allowed after November 16, or during the plan year unless you experience a qualifying life event defined by the IRS. Benefit changes will be effective January 1, 2026.
This is a passive enrollment year, which means employees who are already enrolled do not need to re-enroll unless they want to make a change. The only exception to this is our Flexible Spending Account (FSA) Health Care and Dependent Care.
On Lok Health and Wellness Subscribers: Our 2026 On Lok Health and Wellness plan, managed by Surest, includes an updated plan design.
Pharmacy: CVS CareMark - will replace Optum for prescriptions
Kaiser Permanente Subscribers: Kaiser has an updated plan design including an increase to the HRA funding.
It will be available in the benefits portal by the end of the day on November 3.
The guide covers costs and benefit details.
All three versions are linked here: English, Spanish, and Traditional Chinese, and they are also available in the benefits portal mentioned above.
Printed copies are available at On Lok Centers starting late October.
Linked here is the User Guide with instructions on how to participate in the online open enrollment in UltiPro. Currently, the guide is only available in English.
