Major Milestones Achieved!

We are excited to share that On Lok has reached significant milestones in the development of our On Lok PACE Centers in Union City and South San Jose. Please read below regarding recent activities to grow On Lok’s PACE program and learn more about the timeline leading up to opening our doors.

November 2025

We completed the virtual and onsite state readiness reviews with the Department of Health Care Services (DHCS) Nov 14 – 18. DHCS is the state regulator who must give approval for our new centers, and they did not have any findings during the audits. Passing our reviews with no findings is a significant achievement and is the direct result of the hard work of On Lok staff.

Thank you to the following teams for helping On Lok reach this project milestone: PACE Operations, Transportation, Rehab, Kitchen, Medical, IT, Furniture, Fixtures, and Equipment (FF&E), Facilities, Security, Logistics, Purchasing, Talent, Emergency, Activities, Reception, Accounting, and Enterprise Applications.

November – December 2025

After we receive final DHCS approval, we will submit our applications to the Centers for Medicare and Medicaid Services (CMS) for federal approval. Once we submit, CMS has 45 days to review and approve our applications. In the meantime, On Lok’s Talent Team is interviewing and recruiting staff so we are properly staffed when we open the new centers, and our Marketing, Enrollment and Community Engagement Teams are proactively planning new events and campaigns ahead of opening our new centers to increase On Lok’s visibility in the South/East Bay.

January 2026

Union City will first open as an Alternative Care Setting (ACS) offering clinical services for our On Lok PACE Peralta Center participants. The initial transition to Union City will provide necessary clinic space and office space for staff as we move toward ending our leases at the On Lok Senior Health Services Satellite Clinic (i.e., Mowry clinic) and Fremont Administrative Office on Paseo Padre Parkway in Fremont.

Early 2026

Teams are working diligently to open the On Lok PACE Union City and On Lok PACE South San Jose Centers in early 2026! Our opening will depend on receiving all required state and federal approvals. Once we have a confirmed date, we will share it.

Celebratory Events

Grand opening events are being planned for Summer 2026 so we can celebrate our two new centers with our community. We are also exploring holding small celebratory events for On Lok staff in Spring 2026.
