Today marks the launch of On Lok Check-In, our first employee engagement pulse survey. It takes just 60 seconds to complete, and it is your chance to share how things are going at work right now.

We know that your experience at work doesn't stay the same all year long. Regular check-ins let us hear from you more often so we can understand what is working and where we can do better. Your past feedback has already made a difference. Appreciation in Action launched because you told us recognition matters. Now, we want to keep that momentum going.

The On Lok Check-In:

• Fast: Only 6 questions (approx. 60 seconds).

• Inclusive: Available in English, Spanish, and Chinese.

• Safe: Your responses are strictly confidential.

You can take the survey starting today through February 22, 2026.

How to Participate:

1. Check your Email: Look for a message from Culture Amp with your personal link.

2. Scan a QR Code: Look for flyers in centers and HR offices.

   • Note: If using the QR code, your Employee ID number is your survey code (please do not add leading zeros).
