On Lok PACE South San Jose Center began serving participants earlier this year and expanded access to On Lok PACE services across Santa Clara County, where On Lok remains the only PACE provider serving the entire county.
The celebration brought together members of the extended On Lok family, including community advocates, local officials, construction partners, donors, Board members, and staff whose dedication made this achievement possible.
"Opening two new PACE centers at the same time was ambitious, complex, and extraordinary," said CEO Grace Li during the ceremony. "This achievement reflects the commitment of more than 1,200 employees who carry out our mission every day and help ensure more older adults can access the high-touch, comprehensive care that defines On Lok PACE."
The South San Jose Center opened alongside the new Union City PACE Center earlier this year, representing the first time in On Lok's history that two new PACE centers were launched simultaneously. Together, these centers expand access to care for more than 800 additional older adults throughout the Bay Area.
As the founder of the Program of All-Inclusive Care for the Elderly (PACE), On Lok has spent more than five decades pioneering innovative, community-based care that helps older adults avoid unnecessary nursing home placement while receiving coordinated medical, social, and supportive services.
With the opening of South San Jose and Union City, On Lok now operates nine PACE centers throughout the Bay Area.
The event featured remarks from local elected officials and community leaders, including San Jose Councilmember George Casey, who joined On Lok in recognizing the importance of expanding services for the region's growing older adult population.
The new center also reflects On Lok's continued commitment to advocating for seniors and ensuring access to community-based care options. As demographic shifts continue and demand for services grows, the South San Jose Center positions On Lok to serve more older adults and families closer to home.
Most importantly, the day served as a celebration of the people who made this vision a reality.
From the project teams who planned and built the center, to the clinicians and frontline staff who now welcome participants through its doors each day, the opening represents years of collaboration, dedication, and belief in On Lok's mission.
Together, we are investing in a future where every older adult is seen, valued, and cared for.
As guests toured the new facility and gathered for the ceremonial ribbon cutting, the message was clear: this center is more than a new building. It is a symbol of On Lok's continued growth, its commitment to innovation in aging services, and its promise to serve older adults for generations to come.
